Most ruthless terrorist group and/or crime syndicate?

There is no top dog really. In the united states, the most ruthless to date is MS 13, a salvadorian gang which is notorious for using machete's. Each has their own strengths, there is none that reigns supreme really as they are all different in terms of geographical location.
